Manx Care is introducing a new service to cut down hospital admissions and support earlier discharge.
The Intermediate Care Service aims to reduce the number of people attending A&E, and people in hospital that don't need to be there.
The health body says the redesign of current services will bridge the gap between community and secondary care services.
It means those working with the emergency department, hospital wards and the community can ensure appropriate, time critical assessments can be done through a single point of referral.
