    Report - 'systemic failings' led to woman's death

    A man whose mother was killed by a prisoner who'd been released early from jail says he's glad the government's improving its systems.

    Donovan Kitching killed Gwen Valentine in a car crash as he fled police in April 2014 - he'd left jail weeks earlier.

    Today, a report's been published into how Kitching came to be freed when he'd been described as highly likely to reoffend.

    The document highlights failings in the probation service, prison service, parole committee and police.
